The mechanic budgeted an hour for a lubrication that properly takes more than four. The acme nut quietly wore through 90 percent of its threads — until, off the California coast, the last of them stripped and 88 people went into the Pacific.

An MD-83's pitch trim rides on a single jackscrew in the tail — a rotating acme screw driving a fixed acme nut — and the design's life assumption was grease. Alaska Airlines had stretched the lubrication interval step by step, from every 700 flight hours in 1985 to a time-controlled 8 months — about 2,550 flight hours — by 1996, and the end-play wear check to every 30 months, about 9,550 flight hours, both with FAA acceptance. The last recorded lubrication, 24 September 1999 at San Francisco, was performed by a mechanic who estimated the whole task at about an hour — properly done it takes more than 4 person-hours — and who could not recall seeing fresh grease come out of the top of the acme nut, the only real confirmation the grease went in. The Board found no effective lubrication on the screw-nut interface, only dried and degraded grease in the nut's grease passage where fresh grease should have been, and concluded that more than just that one servicing had been missed or inadequately performed. On 31 January 2000 the nut's remaining threads sheared in cruise; the jackscrew pulled through the nut, the horizontal stabilizer ran to a position no pilot could hold, and the airplane dove into the sea.
The probable cause is a lubrication story — acme nut thread wear from insufficient lubrication — and every contributing cause is an interval story: the extended lubrication interval, and the FAA's approval of it, made a single missed grease job dangerous; the extended end-play check interval, likewise approved, removed the chance to catch the wear before failure; and the MD-80 had no fail-safe behind the threads. After the accident the FAA made both tasks tightly time-controlled — lubrication every 650 flight hours, end-play checks every 2,000 — by airworthiness directive (NTSB AAR-02/01 §1.6.3.2.1, §1.6.3.3.4). The law this card teaches: a servicing task can be the only barrier against a non-survivable failure, and intervals are engineering, not scheduling convenience.
Catastrophic jackscrew thread loss is not quantified as a failure class in public sources — the certification assumption was that lubrication made it remote. The practical margin showed after the accident: Alaska Airlines temporarily grounded 18 of its 34 MD-80s because their end-play checks had been done with nonconforming shop-made fixtures (NTSB AAR-02/01 p. 46).
The acme nut threads stripped in cruise at 31,000 feet. The first dive took the airplane from about 31,050 feet to between 23,000 and 24,000 feet before the crew arrested it; about ten minutes later the remaining structure let go, the airplane pitched to a recorded 70 degrees nose down, and it struck the Pacific 2.7 miles north of Anacapa Island. All 88 aboard — 2 pilots, 3 cabin crewmembers, 83 passengers — were killed; the airplane was destroyed by impact forces (NTSB AAR-02/01 §1.1).
