What happened

PATTERN CARD. Electrical wiring errors during maintenance — crossed connections, wrong pins, unclamped harnesses left to chafe — share one ugly property: the wrong connection usually mates perfectly. In the anchor case, an American Airlines 737-800 completed a scheduled steel-to-carbon brake modification; the electrical connectors for the number 1 and number 2 left-gear wheel speed transducers were swapped, and two right-gear flexible hydraulic hoses were reversed at the flow limiters. Nothing looked wrong, everything fit, and there was no functional check able to catch either swap. Four days later, landing at DFW, the anti-skid system worked exactly backwards: skidding wheels got full brake pressure, rolling wheels got the release. The outboard tires wore flat and failed, stopping distance stretched past the runway end, and the aircraft came to rest in the paved overrun area. All 104 aboard evacuated uninjured. The NTSB noted it has investigated several incidents and accidents from cross-wired wheel speed transducers — all of them maintenance human error.

Why it matters

The probable cause is a two-line indictment of process, not of one technician: improper maintenance due to human error during a braking system modification, with the lack of a functional check to verify hoses and transducer wiring contributing (NTSB DCA24LA095). The pattern is old and named: the UK CAA's 1992 analysis of maintenance occurrence reports lists electrical wiring discrepancies — including cross-connections — among the most frequently recurring maintenance errors, alongside incorrect installation and wrong parts (UK CAA 1992 MOR analysis, reproduced in CAP 716 Appendix D). Where wiring is safety-critical, the defense is never that the connector reached: it is labeling before disconnection, verification against the diagram, and a functional test designed to expose a swap.

Control sequence
  1. Label harnesses and connectors before disconnection — identical connectors on adjacent lines are a cross-connection waiting for a shift change.
  2. Reconnect from the wiring diagram and the drawing, never from reach, routing memory or "it only fits one way" — it usually fits both ways.
  3. Clamp and route per the drawing; an unclamped harness that merely touches structure today is a chafed short circuit later.
  4. After any wiring disturbance on a safety system, run a functional/operational check that is actually capable of detecting a cross-connection — a continuity beep proves the wire, not the system.
  5. Put an independent inspection on reconnected flight-safety wiring before release; the installer's eye is the one eye guaranteed to see what was intended, not what is.
How often

Electrical wiring discrepancies, including cross-connections, appear in the UK CAA's 1992 listing of the most frequently recurring maintenance errors (UK CAA 1992 MOR analysis, reproduced in CAP 716 Appendix D); the NTSB states it has investigated several incidents and accidents resulting from cross-wired wheel speed transducers, all the result of maintenance human error (NTSB DCA24LA095).

Consequences

In the anchor case the reversed anti-skid released the wrong brakes and locked the skidding wheels; the outboard tires failed, braking effectiveness collapsed, and the 737 overran runway 17L into the paved overrun area at DFW. No injuries among the 104 aboard; minor aircraft damage. American Airlines subsequently inspected 303 of its 737s — none had swapped wiring — and Boeing issued fleet-wide notices and revised service bulletins on hose and transducer reconnection (NTSB DCA24LA095, safety actions).

Evidence gate: publish only after verification
