What happened

Smart pumps carry a dose-error-reduction system (DERS) — a drug library with hard and soft dose limits. Programming in 'basic' or non-DERS mode, or bypassing drug selection, skips those limits entirely, so a dangerous dose meets no guardrail.

Why it matters

The Joint Commission (SEA #63, 2021) reports that bypassing the pump or the drug library accounted for about 10% of infusion-administration errors, and ISMP calls for organizations to reach over 95% DERS compliance. Control sequence
  1. A smart pump offers a drug library (DERS) with dose limits and a basic / non-DERS mode without them.
  2. Under time pressure, the clinician selects basic mode or skips drug selection.
  3. The dose limits that would flag an unsafe entry are bypassed.
  4. A dangerous dose is programmed with no guardrail to catch it.
  5. The pump delivers exactly what was entered.
