A pipe carrying hot hydrocarbon has been quietly thinning from corrosion and ruptures without warning, dumping a large flammable volume — often above auto-ignition temperature — into the open. A vapour cloud forms in seconds with workers inside it.
Chevron Richmond 2012 proved the wall thinning was inspectable and predictable — the failure was a mechanical-integrity/inspection gap, not bad luck. Trainees must learn both halves: act on marginal condition flags before failure, and respond correctly when the pipe lets go anyway.
