Corroded Pipe Rupture — Sudden Large Release

The wall had been thinning for years. It let go all at once.

What happened

A pipe carrying hot hydrocarbon has been quietly thinning from corrosion and ruptures without warning, dumping a large flammable volume — often above auto-ignition temperature — into the open. A vapour cloud forms in seconds with workers inside it.

Why it matters

Chevron Richmond 2012 proved the wall thinning was inspectable and predictable — the failure was a mechanical-integrity/inspection gap, not bad luck. Trainees must learn both halves: act on marginal condition flags before failure, and respond correctly when the pipe lets go anyway.

Control sequence
  1. (Field/what-if) Act on the marginal/overdue thickness flag — take the line out of service before failure.
  2. On rupture: get people out of the line of fire immediately.
  3. Activate ESD / remote isolation to cut the source feed.
  4. Confirm blowdown/depressuring of the ruptured segment.
  5. Escalate and keep ignition sources controlled before the pre-set ignition point.
